Moving Health Literacy Together: Module 4 - Navigating Health Care Teams & Health Literacy Resources

Webinar

Description:
Moving Health Literacy Together is a four-part training series that aims to equip community health workers (CHWs), physicians, and health care team members with a comprehensive understanding of health literacy and its crucial role in improving health outcomes.  Although designed to be taken in order, it is not a requirement and you may choose to take one or more modules.

Module 1 covers the importance of health literacy in addressing health disparities, the social and cultural factors that influence it, and how professionals can advance health literacy within their communities.  Module 2 explores health literacy's impact on health outcomes as a social determinant of health.  In Module 3, participants will learn effective strategies for screening clients/patients for health literacy, engaging in meaningful discussion about it, and integrating culturally appropriate, literacy-friendly health education strategies into practice.  Module 4 highlights the essential role of CHWs in this work as integral members of the healthcare team, as well as identifying key resources for health teams and clients/patients.

Module 4 - Navigating Health Care Teams and Relevant Health Literacy Resources

  • Identify the role of CHWs on health care teams
  • Implement strategies to navigate team dynamics and culture
  • Access and apply health information and related literacy resources
